Policies
Whenever I get a new client, I have them fill out an intake form. This consists of standard medical questions that are kept strictly confidential according to HIPAA rules. This form lets me make better informed decisions regarding type (or types) of massage to use without exacerbating any existing medical conditions you may have.
Also, clients must fill out a Clinic Policy form on their first visit. Here are some highlights:
Massage therapy and bodywork services are a therapeutic aid and are not to be construed in any sexual way.
Your privacy is assured and you need only to disrobe to your level of comfort.
You may not be intoxicated during your session. If intoxication is believed, your session will not start and you will still be charged for the full time. Also, cleaning vomit off massage equipment is not fun.
Payment, whether through cash, credit card, or gift certificate (sorry, no checks are accepted), is expected at each appointment, unless an alternate payment option has been agreed upon prior to my arrival.
A parent or legal guardian is required to sign the consent form before massage can be given to any person under the age of 18.
Cancellations
Unless you have an emergency, cancellations MUST be given with at least a 12-hour advance notice. If 12 hours advance notice is not given, 50% of the cost of treatment will be billed to you. To cancel any appointment, please call 224-703-8380. If I am not available to answer, please leave a message.
